Quote requests submitted through our website are not binding orders. We will confirm availability, pricing, and any prescription requirements before finalizing any purchase. All orders are subject to product availability and acceptance by Zivan Medical Supply.

Certain medical equipment requires a valid prescription from a licensed healthcare provider. We reserve the right to request prescription documentation before fulfilling any order. By ordering prescription-required items, you confirm that you have or will obtain the necessary prescription.

If you submit insurance information for coverage verification, you understand that insurance approval is determined solely by your insurance provider. We provide assistance with the verification process but cannot guarantee coverage or reimbursement.
